How to Register

Anyone living in our practice Catchment area can join. Regardless of age, personal circumstances or medical problems

It is two steps procedure to  register with our practice

Step 1:  Follow the link below to complete the online registration form and patient information details.

Step 2: Either download the GMS1 form below or collect the paper form from the practice.

After submitting your online registration form please complete the GMS1 Form and  drop in  GMS1 form in person. please remember to take a photo ID and proof of address with you so reception staff can verify your details .

***For Children under 16 we need a copy of vaccination from RED BOOK

New Patient Health Check

Once registered, make an appointment with our health care assistant for a new patient health check.

We invite all our new patients to see one of our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.

As a patient you can choose to see any of the doctors. However, we recommend that you try to establish a relationship with one doctor, who you come to regard as your usual doctor, to aid continuity and quality of care.

We are a non-limited partnership NHS practice. Contracted to provide general medical services by NHS England Commissioning Board.

We register anyone eligible for NHS treatment who is living in our catchment area. Or extended boundary catchment area. We would be delighted to register all persons or family members living at your address in order to offer full care.

Moving Home?

Re-Registering

If you move home outside our catchment area you will need to re-register with a GP local to where you live. We are not able to provide general medical services, including home visits, once you have moved outside our Catchment area
